1.1.  Complete the official application

2.2.  Have a check or money order made out to the “Secretary of State" in the amount of $40.00

3.3.  A notary public applicant must submit a  2”X 2” color passport photograph of himself/herself to the Secretary of State along with the application.

4.4.  Produce one of the following types of identification:

 

a)Driver's license
b)State issued identification card
c)Passport
d)Active duty military ID
