3.  Subscribing Witness
 
 
The notary gives the oath to the subscribing witness and asks the following questions:
 

1.1.  Do you personally know the principal?

2.  Did you witness the principal sign the document or did the principal acknowledge signing the document?

3.  Did the principal ask you to sign the document as a witness?

 

The subscribing witness must sign the document.
