IF THE
NOTARY CHANGES HIS/HER MAILING ADDRESS, OR PRINCIPAL PLACE OF
BUSINESS OR RESIDENCE, THE NOTARY:
Shall notify
the Secretary of State by certified mail within
30 days of the
change of address, indicating the
1. correct name,
2. commission number,
3. commission expiration date,
4. new
address,
and whether the address change is for the business,
residence, and/or for mailing purposes

A
notary public may not use a commercial mail receiving agency
or post office box as his or her principal place of business
or residence unless the notary public provides the Secretary
of State with a physical street address as the principal
place of residence.
2.
Willful failure to notify the Secretary of State of a change
of address is now punishable as an infraction by a fine of
up to $500.
The notary may
file a new oath and bond and request a new official seal when
transferring principal place of business.
1.If
the notary public changes his/her name, he/she shall complete an
application for a name change form and file that application
with the Secretary of State.
Name changes; application; filing. Willful failure to notify

term of the commission and the commission number shall remain
the same.
